First Visit

 

Thank you for choosing Dr. Russell O. Schub, P.A. as your partner in your health care needs. Appointments can be scheduled by calling 410-730-1000, ext.0.

Please bring the following items to your first appointment:
 

  • Completed Patient forms (Click here to download the forms).
  • Insurance Card
  • Referral, if needed
  • Picture ID
  • Current labs and testing pertinent to your visit.
  • A list of your current medications, including the dosages and frequency.

If for any reason you are unable to keep your appointment, please give us 24 hours notice. We charge a fee for missed appointments that are not cancelled within 24 hours, and procedures that are not cancelled within 72 hours of the scheduled procedure. Our office makes every effort to contact you prior to your appointment.
